Jennifer Nacole Simmons Womack, age 35, of Pleasant Grove passed away September 17, 2017. She was preceded in death by her grandfather, Garrett C. Jones; grandmother, Katie Lanell Simmons. She is survived by her daughter, Haiden Womack; mother, Jennifer Simmons (Tommy); sister, Natalie Smith (Terry); brother, Jacob Simmons; grandparents, Doris Jones, Jimmy Simmons Funeral services will be Wednesday, September 20, 2017 at 2:00 P.M. at Patterson-Forest Grove Funeral Home with burial to follow in Forest Grove Memorial Gardens. Rev. Daniel Wilson officiating. Visitation will be Wednesday, at 1:00 P.M. at the funeral home.
